POSITION DESCRIPTION
The Registered Nurse renders care in the Home Health Care Center and Hospice and is responsible for all the activities under that department in skilled nursing services, in addition to those activities listed under Hospice policies. This individual cooperates with other members of the Home Health CareCenter and Hospice team and the departments of the hospital(s). This individual assesses patient/client needs, develops the plan of care including education, and revises that plan of care as directed by the rules and regulations of state, federal and JCAHO organizations. This individual is also responsible for maintaining the quality of care as directed by standards of care and for maintaining his/her competencies.

REQUIREMENTS
Education Needed
Must be a graduate of an accredited school of nursing with CPR certification and a valid Indiana State RN license. A BSN is preferred.


Experience Needed
Must have a minimum of one year acute care (or equivalent) or public health experience. A Home Health Care and Hospice background is preferred.
