POSITION DESCRIPTION
Plans and implements specific treatment programs for individual patients according to the principles and Practices of physical therapy. Collaborates with interdisciplinary treatment team.

Education Needed
Minimum of Bachelor of Science in Physical Therapy from an accredited program and must have or have applied for, licensure in the State of Indiana.

Required Certification/Registration
Licensure in the State of Indiana, CPR Certification

Graduate of accredited program
